Annual Service Reports

All ministers of The Wesleyan Church are required to submit an Annual Service Report to the District Board of Ministerial Development at the time designated by the district (May 10).

Please follow these instructions for downloading the report:

1. Open the correct form

2. Fill out form/Save to your computer

3. E-mail your completed form to pjdistrict@aol.com
